The public Apply page form currently collects: first name, last name, email address, phone number, state, ZIP code, preferred contact method, eligibility basis, device preference, household acknowledgement, contact and privacy consent, an optional general message, a hidden form-start timestamp, a bot-detection honeypot field and a Cloudflare Turnstile response token.
The public form does not request SSN, dates of birth, identity documents, full residential addresses, benefit account numbers, bank or payment details, or file uploads. Do not include those details in the optional message field, email messages or any public website form.
Submitted information may be used to respond to an application inquiry, contact the applicant, discuss possible eligibility preparation, discuss device or service availability, prevent fraud and abuse, maintain operational records, and comply with applicable legal requirements.
Information may be processed through configured services such as Cloudflare Pages Functions, Cloudflare Turnstile, the configured secure webhook or CRM if enabled, an approved email-delivery provider such as Resend if enabled, and hosting or security providers. This policy does not claim a specific CRM is active unless it is actually configured.
Information is retained only as long as reasonably needed for application assistance, support, operational, security and legal purposes. Air Free Phone has not published a fixed retention period because no client-approved fixed period is available in this repository.
A user may request access, correction, deletion or withdrawal of contact consent by emailing support@airfreephone.com, calling 504-331-0527 or calling 800-728-9561. Requests may require reasonable identity verification and may be subject to legal or operational limitations.
This policy does not publish a stronger statement about personal-information sales or advertising sharing because that specific policy has not been confirmed in the repository. Privacy or marketing-use questions may be sent to support@airfreephone.com.
The public application form is not intended for independent submission by children. A parent, guardian or responsible adult should handle household application inquiries where appropriate.
Air Free Phone uses reasonable safeguards such as HTTPS form submission, Cloudflare Turnstile abuse protection and server-side validation. No internet transmission or electronic storage system can be guaranteed perfectly secure.
